There is no generally accepted definition of this form of pathology for both echocardiography and cardiac magnetic resonance imaging. Non-compact left ventricular myocardium can occur at any age and is often asymptomatic. Treatment of non-compact left ventricular myocardium is nonspecific and symptomatic.</p> <p><bold>DESCRIPTION</bold>: This study describes a clinical case of a young patient who was diagnosed with non-compact left ventricular myocardium. The disease debuted in the form of cardiac arrhythmia. Holter monitoring showed frequent ventricular extrasystole and atrioventricular block of the first and second degrees. Moreover, echocardiography revealed a decrease in global contractile function of the left ventricle. The diagnosis was made after magnetic resonance imaging of the heart, which revealed the presence of non-compact myocardium of the anterior, lateral, and inferior walls of the left ventricle in the apical and middle segments. Sustained rhythm disturbances were not induced in electrophysiological research by rapid and programmed stimulation. The patient was prescribed antiarrhythmic therapy, followed by echocardiography Holter monitoring. In the future, the patient needs regular monitoring by a cardiologist.</p> <p><bold>CONCLUSION</bold>: Non-compact left ventricular myocardium should be diagnosed at an early stage, so that life expectancy can be increased owing to timely treatment of heart failure and the use of oral anticoagulants, antiarrhythmic drugs, cardiac resynchronization therapy, cardioverter-defibrillator implantation, and heart transplantation when other treatment options are ineffective.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование</bold>. Некомпактный миокард левого желудочка — редкая гетерогенная патология, в основе которой лежит двуслойное строение миокарда. Общепринятое определение данной формы патологии отсутствует как для эхокардиографии, так и для магнитно-резонансной томографии сердца. Патология может встречаться в любом возрасте и часто протекает бессимптомно. Лечение неспецифичное и симптоматическое.</p> <p><bold>Описание клинического случая</bold>. В данной публикации мы представляем описание клинического случая некомпактного миокарда левого желудочка, который был диагностирован у пациентки молодого возраста. Заболевание дебютировало клиникой нарушения ритма сердца, по результатам холтеровского мониторирования зарегистрирована частая желудочковая экстрасистолия, атриовентрикулярная блокада I и II степени. При эхокардиографии обращало на себя внимание снижение глобальной сократительной функции левого желудочка. Установить диагноз удалось после проведения магнитно-резонансной томографии сердца, в результате которой было выявлено наличие некомпактного миокарда передней, боковой и нижней стенок левого желудочка в верхушечных и средних сегментах. При электрофизиологическом исследовании методами учащающей и программируемой стимуляции устойчивые нарушения ритма не индуцировались. Пациентке была назначена антиаритмическая терапия с последующим контролем холтеровского мониторирования электрокардиограммы и регулярным наблюдением кардиолога в дальнейшем.</p> <p><bold>Заключение</bold>. Крайне важно диагностировать некомпактный миокард левого желудочка на ранней стадии, благодаря чему продолжительность жизни может быть увеличена за счёт своевременно начатого лечения сердечной недостаточности, применения пероральных антикоагулянтов и антиаритмических препаратов, сердечной ресинхронизирующей терапии, имплантации кардиовертера-дефибриллятора и трансплантации сердца, когда другие варианты лечения неэффективны.</p></trans-abstract><kwd-group xml:lang="en"><kwd>non-compact myocardium of the left ventricle</kwd><kwd>ventricular extrasystole</kwd><kwd>echocardiography</kwd><kwd>Holter monitoring of the electrocardiogram</kwd><kwd>magnetic resonance imaging</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>некомпактный миокард левого желудочка</kwd><kwd>желудочковая экстрасистолия</kwd><kwd>эхокардиография</kwd><kwd>холтеровское мониторирование электрокардиограммы</kwd><kwd>магнитно-резонансная томография</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Hirono K, Ichida F.
