All examinations were conducted in different expert institutions in Moscow and the Moscow region in the period from 2019–2023. A content analysis of regulatory legal documents regulating the activities in the provision of medical care to patients with an emergency injury has been performed. A correlation was made between the unfavorable outcome and the identified defects in the provision of medical care to patients with acute injuries. Due to the fact that these medical care defects are nominal, we have applied Pearson's χ<sup>2</sup>.</p> <p><bold>RESULTS: </bold>In the analysis of expert opinions, in most cases, defects in the provision of medical care were identified — 137 (91.3%), of which 47 (31.3%) conclusions contain 2 or more defects simultaneously. In 49 cases (32.7%), the expert assessment of the nature of the damage and the treatment plan was influenced by a defect in the management of medical records; in 38 cases (25.3%) — defects in treatment; in 27 cases (18.0%) — defects in diagnosis; in 19 cases (12.6%) — defects in the organization of medical care. Diagnostic defects related to missed fractures were detected in 18 (12.0%) cases. Most of the diagnostic defects were noted at the outpatient stage of medical care provision in a trauma center. Among the reasons for incorrect diagnosis, the following can be distinguished: inattentive examination of the victim, incorrect description of radiographs by a radiologist or a later description (for the next day or more), the inability to perform more high-tech and other research methods other than X-ray, for example, computed tomography or ultrasound.</p> <p><bold>CONCLUSION: </bold>In emergency traumatology, the probability of an adverse outcome is most affected by treatment defects (the relative risk was 1.21 with 25.3% of treatment defects — from 0.776 to 2.307). In the specialty "traumatology and orthopedics", there is a need to create scientifically sound, relevant to modern requirements of medical activity, clinical guidelines for patients with injuries of the musculoskeletal system, as well as to develop protocols for the provision of medical care to patients with polytrauma.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> По данным литературы, отмечается неукоснительный рост обращений граждан с жалобами на качество оказания медицинской помощи (МП).</p> <p><bold>Цель.</bold> Выполнить анализ заключений судебно-медицинских экспертиз наиболее проблемных с точки зрения правового риска групп пациентов с острыми травмами опорно-двигательного аппарата; провести корреляцию между неблагоприятными исходами и распространёнными дефектами оказания МП, имеющими значение при правовой оценке действий врача-травматолога; а также определить организационные мероприятия для улучшения качества оказания МП.</p> <p><bold>Методы.</bold> Выборочно проанализированы 150 заключений комиссионных судебно-медицинских экспертиз. Все экспертизы проведены в разных экспертных учреждениях Москвы и Московской области в период 2019–2023 гг. Выполнен контент-анализ нормативно-правовых документов, регламентирующих деятельность при оказании МП пациентам с экстренной травмой. Определена корреляция между неблагоприятным исходом и выявленными дефектами при оказании МП пациентам с острыми травмами. В связи с тем, что данные дефекта МП являются номинальными, нами применён критерий χ<sup>2</sup> Пирсона.</p> <p><bold>Результаты.</bold> При анализе экспертных заключений в большей части случаев — 137 (91,3%) — выявлены дефекты оказания МП, из них в 47 заключениях (31,3%) фигурируют 2 и более дефекта одновременно. В 49 случаях (32,7%) на экспертную оценку характера повреждений и плана лечения повлиял дефект ведения медицинской документации; в 38 случаях (25,3%) — дефекты лечения; в 27 случаях (18,0%) — дефекты диагностики; в 19 случаях (12,6%) — дефекты организации МП. Дефекты диагностики, связанные с пропущенными переломами, были выявлены в 18 случаях (12,0%). Большая часть дефектов диагностики отмечалась на амбулаторном этапе оказания МП в условиях травматологического пункта. Среди причин неправильной диагностики можно выделить следующие: невнимательный осмотр пострадавшего; неправильное или позднее (на следующие сутки и более) описание рентгенограмм врачом-рентгенологом; невозможность выполнения более высокотехнологичных и других методов исследования, кроме рентгенологического, например компьютерной томографии или ультразвукового исследования.</p> <p><bold>Заключение.</bold> В экстренной травматологии на вероятность наступления неблагоприятного исхода больше всего влияют дефекты лечения (относительный риск составил 1,21 при 25,3% дефектов лечения — от 0,776 до 2,307).
