Hypertension, when concomitant with CTTH, may affect the clinical course of the disease and reduce treatment efficacy. Most studies investigating the prevalence of combined headache and hypertension have shown a higher occurrence of hypertension in patients with CTTH compared with those suffering from episodic tension-type headache. However, age-specific features of these associations remain insufficiently studied.</p> <p><bold>AIM:<italic> </italic></bold>This study aimed to investigate age-related clinical features of CTTH in patients with concomitant hypertension.</p> <p><bold>METHODS:<italic> </italic></bold>A single-center retrospective–prospective observational study was conducted, consecutively enrolling 97 patients diagnosed with CTTH (International Classification of Headache Disorders, 3rd edition, 2018) who consulted a neurologist in 2019–2023. No medical interventions were performed. Patients were divided into two age groups: 18–44 years (group 1, young adults) and 45–74 years (group 2, middle-aged and older adults). Pain intensity, number of headache days, pericranial muscle dysfunction on manual examination, provoking factors, presence of hypertension, and current emotional state (assessed by Hamilton and Beck screening scales) were evaluated. Statistical analysis was performed using SPSS Statistics 26.0; <italic>p</italic> &lt; 0.05.</p> <p><bold>RESULTS:<italic> </italic></bold>Hypertension was detected in 32% of patients with CTTH: 9.6% in group 1 and 57.7% in group 2 (<italic>p</italic> &lt; 0.001). Among patients aged 18–44, pain intensity was higher in those with hypertension compared to those without (<italic>p</italic> = 0.0463). In patients aged 45–74, hypertension was associated with a higher number of headache days compared to peers without hypertension (<italic>p</italic> = 0.0281). Pericranial muscle dysfunction was observed in 93.8% of examined individuals, with more pericranial muscles affected in group 2 (<italic>p</italic> = 0.042). Isolated involvement of a single muscle was more common in patients aged 18–44 (<italic>p</italic> &lt; 0.001). Specifically, in younger patients without concomitant hypertension, significant involvement of the right trapezius muscle was observed (<italic>p</italic> = 0.029). In patients aged 45–74, regardless of hypertension status, combined pericranial muscle involvement was more frequent than isolated dysfunction (<italic>p</italic> = 0.019).</p> <p><bold>CONCLUSION:<italic> </italic></bold>CTTH in patients of different ages with concomitant hypertension demonstrates distinct clinical features, underscoring the need for personalized therapy.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование. </bold>Хроническая головная боль напряжения (ХГБН) — распространённая патология, встречающаяся в различных возрастных группах. Артериальная гипертензия (АГ), сопутствующая ХГБН, может оказывать влияние на клиническое течение заболевания и снижать эффективность терапии. Большинство проведённых исследований, посвящённых изучению распространённости сочетания головной боли и АГ, показали более частую встречаемость АГ у пациентов с ХГБН по сравнению с лицами, страдающими эпизодической формой головной боли напряжения. Однако возрастные особенности этих взаимосвязей остаются недостаточно изученными.</p> <p><bold>Цель. </bold>Изучить возрастные особенности клинической картины ХГБН у пациентов с сопутствующей АГ.</p> <p><bold>Методы. </bold>Проведено одноцентровое ретроспективно-проспективное наблюдательное исследование с последовательным включением 97 пациентов с диагнозом ХГБН (Международная классификация головной боли, 3-е издание, 2018), обратившихся к неврологу в 2019–2023 гг. Медицинское вмешательство не осуществлялось. Обследуемые распределены на две возрастные группы: пациенты 18–44 лет — группа I (молодой возраст); пациенты 45–74 лет — группа II (средний и пожилой возраст). Оценивали интенсивность боли, число дней с цефалгией, дисфункцию перикраниальных мышц при мануальном обследовании, провоцирующие факторы, наличие АГ, текущее эмоциональное состояние (по скрининговым шкалам Гамильтона и Бека). Статистическая обработка выполнена в программе SPSS Statistics 26.0, <italic>p</italic> &lt;0,05.</p> <p><bold>Результаты. </bold>АГ выявлена у 32% пациентов с ХГБН: у 9,6% — в I группе, у 57,7% — во II группе (<italic>р</italic> &lt;0,001). У пациентов 18–44 лет при наличии АГ интенсивность боли была выше, чем у лиц без АГ (<italic>р</italic>=0,0463). У пациентов 45–74 лет с АГ отмечено большее число дней с головной болью, чем у сверстников без АГ (<italic>р</italic>=0,0281). Дисфункция перикраниальных мышц зарегистрирована у 93,8% обследованных лиц, с преобладанием большего числа вовлечённых перикраниальных мышц во II группе (<italic>p</italic>=0,042). Изолированное поражение одной мышцы чаще встречается у лиц 18–44 лет (<italic>р</italic> &lt;0,001). В частности, у пациентов молодого возраста без сопутствующей АГ выявлено статистически значимое вовлечение трапециевидной мышцы справа (<italic>p</italic>=0,029).
