Despite the high prevalence of this problem, data on injury patterns, seasonal variability, and clinical and organizational aspects of care for this patient population in the setting of emergency hospitalization remain limited.</p> <p><bold>AIM: </bold>To assess the distribution and clinical characteristics of traumatic injuries in older patients admitted on an emergency basis; to identify factors influencing the course and outcomes of traumatic disease; and to analyze treatment strategies depending on the severity of comorbid conditions.</p> <p><bold>METHODS: </bold>A retrospective, single-center, cohort study was conducted. The analysis included 3588 patients aged ≥ 56 years who were admitted to the trauma department of a multidisciplinary hospital (Moscow) between 2021 and 2024 for an emergency.</p> <p><bold>RESULTS: </bold>Women accounted for 67.9% of the cohort (<italic>n</italic> = 2438). The mean patient age was 72.74 ± 10.0 years. The highest number of hospital admissions occurred during the winter season (32.3%). Low-energy fractures predominated, most commonly fractures of the proximal femur (33.18%) and the humerus (17.06%). Comorbid conditions were identified in 85.9% of patients, most frequently hypertension (35.5%) and chronic heart failure (19.2%). The mean length of hospital stay was 10.7 ± 9.2 days and increased with older age and in cases of polytrauma. The primary outcome, in-hospital mortality, was 5.3% (95% confidence interval, 4.6–6.1); a total of 190 patients died (135 women and 55 men).</p> <p><bold>CONCLUSION: </bold>The findings confirm a high prevalence of severe osteoporotic injuries among older patients requiring emergency hospitalization. Timely surgical intervention and a multidisciplinary approach were associated with improved clinical outcomes.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование. </bold>Увеличение численности пожилого населения приводит к росту частоты травм, преимущественно остеопоротических переломов, что накладывает значительное бремя на систему здравоохранения. Несмотря на высокую распространённость данной проблемы, до настоящего времени сохраняется дефицит данных по структуре, сезонной вариативности и клинико-организационным аспектам оказания помощи данной категории пациентов в условиях экстренной госпитализации.</p> <p><bold>Цель. </bold>Оценить структуру и клинические особенности травматизма у пожилых пациентов, поступивших по экстренным показаниям; определить факторы, влияющие на течение и исход у них травматической болезни; провести анализ лечебной тактики в зависимости от тяжести коморбидной патологии.</p> <p><bold>Методы. </bold>Проведено ретроспективное одноцентровое когортное исследование. В анализ включены 3588 пациентов в возрасте ≥56 лет, экстренно госпитализированных в травматологическое отделение многопрофильного стационара (Москва) в 2021–2024 гг.</p> <p><bold>Результаты. </bold>Женщины составили 67,9% выборки (<italic>n</italic>=2438), средний возраст пациентов — 72,74±10,0 года. Наибольшее число госпитализаций зарегистрировано зимой (32,3%). Преобладали низкоэнергетические переломы: проксимального отдела бедренной кости (33,18%) и плечевой кости (17,06%). Сопутствующие заболевания выявлены у 85,9% больных, чаще всего — артериальная гипертензия (35,5%) и хроническая сердечная недостаточность (19,2%). Средняя длительность госпитализации составила 10,7±9,2 сут, увеличиваясь с возрастом и при политравме. Основной исход — внутрибольничная летальность — составил 5,3% (95% доверительный интервал 4,6–6,1): умерли 190 пациентов (135 женщин и 55 мужчин).</p> <p><bold>Заключение. </bold>Результаты исследования подтвердили высокую распространённость тяжёлых остеопоротических травм у пожилых при экстренной госпитализации.
