THE APPLICATION OF THERMOGELIOX WITH NEBULIZER THERAPY IN COMPLEX TREATMENT OF PATIENTS WITH BRONCHIAL ASTHMA

Abstract

The article deals with the assessment of effectiveness of application of heated oxygen helium mixture (thermogeliox) as combined with the nebulizer therapy in complex treatment of patients with bronchial asthma. The examined sample consisted of 80 patients with bronchial asthma of moderate and severy degree at exacerbation phase of disease. The complex treatment with thermogeliox was applied to 40 patients of main group. The single session of combined application of thermogeliox and nebulizer therapy to patients with bronchial asthma reliably improves the bronchial patency and results in significantly increases saturation of blood with oxygen in patients with bronchial asthma of severe degree. The complex treatment with thermogeliox of patients with bronchial asthma of moderate and seveiy degree contributes to more evident improvement of clinical picture of disease as compared with standard treatment: reliable decrease of attacks of suffocation and stertor, relief of cough and dyspnea, improvement of indicators of pulmonary ventilation with reliable decrease of intensity of bronchial obstruction, significant increase of partial tension of blood oxygen and blood oxygenation. The highest effectiveness of complex treatment is noted in patients with severe form of bronchial asthma. Besides, a significant improvement of quality of life occurs against the background of tangible improvement of clinical functional indicators. The study results demonstrate the expedience of widespread application of thermogeliox combined with the nebulizer therapy for the purpose of increasing the effectiveness of rehabilitation treatment of patients with bronchial asthma.

THE RESPIRATORY SYSTEM CONDITION UNDER OBESITY

Abstract

The article deals with the study of respiratory system condition in patients with obesity of different mass degree. In the main group the medium and average invert correlation was derived between body mass index, waistline, index of waist volume/hip volume and ventilation indicators. The lowest indicators were established in males with third degree obesity. In this group the VC, FVC, EV1 and PEF were reliably lower than in males of control group. The same group demonstrated a high correlation between waistline, index of waist volume/hip volume, FVC, MEF50, FEV1 and FEF 25-75. The microscopy revealed that in all cases the main pathologic alterations in muscle tissue manifested as separation of myofibrils and disappearance of transversal banding, thinning and destruction of separate fibers with replacement of tissue fragments with adipose cells. The conclusion is made that the intensity of dysfunction of pulmonary ventilation depends on the degree and type of obesity.

Abstract

При морфологическом исследовании секционного материала мышечной ткани диафрагмы и скелетной мускулатуры при ожирении выявлен ряд изменений в структуре волокон и системе кровообращения, особенно в микроциркуляторном русле. Во всех случаях основные патологические изменения в мышечной ткани проявлялись разволокнением миофибрилл и исчезновением поперечной исчерченности, истончением и деструкцией отдельных волокон с замещением фрагментов ткани жировыми клетками. Особенно это хорошо заметно в области микроциркуляторного русла, где сформировались островки жировой ткани, прослаивающие мышечные волокна, что, в свою очередь, приводило к снижению сократительной функции дыхательной мускулатуры.

Abstract

The resistance of respiratory tracts in tobacco smoking patients with persistent bronchial asthma and healthy ones is studied primordially, right away after single smoking of one cigarette and in recovery period to assess the sensitivity to tobacco smoke using the technique of brief interruption of air flow. In patients with bronchial asthma as compared with healthy smokers a more apparent and rapid increase of respiratory tracts resistance because of obstruction presence and more apparent respiratory tracts inflammation is established. Besides, other differences of response reaction of respiratory system to the "acute" tobacco smoking in main and control groups are revealed.

THE STRUCTURE OF POST-INJECTION COMPLICATIONS IN PATIENTS WITH DRUG ADDICTION IN SURGERY DEPARTMENT

Abstract

The article deals with the actual issues of diagnostics and treatment of complications of parenteral drug addiction in conditions of surgery department. The sample of 517 cases of visiting doctors because the complications after parenteral introduction of handicraft narcotic drugs developed was analyzed. The classification of post-injection complications is developed considering the clinical symptomatology, treatment policy and rehabilitation prognosis within each form of complication. It is established that the occurrence of tissue ischemia against the background of parenteral introduction of drug substitutes is related very often not only to the main arteries thrombosis but to the spasm of arterioles and capillaries. The algorithm of diagnostics and treatment of post-injection complications in patients with drug addiction is developed.

Abstract

In patients with diabetes mellitus type I and primarily detected diabetes mellitus type II the metabolic deficiency inherently occurs of aerobic synthesis of ATP as a result of of glycolytic processes activation, lactate concentration increase and lipid peroxidation development. The misbalance in the lipid peroxidation-antioxidant defense system against the background of dyshyperlipemia and lactate concentration increase testifies the development of oxidative stress, especially marked under diabetes mellitus type II. The degree decrease of glucose utilization by erythrocytes and inhibition of activity of K+, Na +-ATPase indicate the desensitivity of cells to insulin. The alterations of under study indicators depend on diabetes type and duration of disease course. The findings reflect the manifestation of common adaptive mechanisms in disease development which promote the adaptation to the negative impact of active forms of oxygen.

THE SCREENING OF HIV-POSITIVE PATIENTS SUFFERING OF ALCOHOL ABUSE AND ITS SIGNIFICANCE IN THE CURATIVE PREVENTIVE WORK

Abstract

The techniques of questionnaire and testing were applied to the sampling of 132 HIV-positive patients. The study revealed that persons with alcohol abuse consist from 32.6 to 53.8%. The clinically reliable symptoms were established among 29.6% of patients (19.7% of females and 39.4% of males). In favor of alcohol abuse indirectly testify tobacco smoking and diseases of liver (hepatitis, cirrhosis) and proper pathologic motivation of alcohol abuse. The HIV-positive patients suffering of alcohol abuse have worse prognosis of course of disease and they are less committed to anti-retrovirus treatment being the target risk group of infection spread.

HELICOBACTER PILORI AND BRONCHIAL ASTHMA IN CHILDREN

Abstract

The results of clinical laboratory, endoscopic and morphologic analyses in children with bronchial asthma and concurrent chronic gastritis demonstrated that the level of total IgE has no significant differences in groups of patients with different degree of Helicobacter contamination and with the absence of Helicobacter pilori. At that, in 78% of patients the IgE level significantly exceeded the normal values. In case of absence of Helicobacter pilori, the lymfo-follicular hyperplasia of mucous membrane of the stomach was detected only in 12% of patients and in presence of Helicobacter pilori in 58.3% of patients. This case is probably related to the intensity of local immune response to Helicobacter infection. In the group of children with bronchial asthma and Helicobacter pilori high contamination the exacerbation of chronic gastritis proceeded in a more severe form.

THE SPEED METHOD AND QUALITY: ARE THEY COMPATIBLE IN DISTRICT THERAPEUTIST WORK?

Abstract

The present system of financing of polyclinics the remuneration of labor of district therapeutists depending on number of received patients and related to condition the regulation of 15 minutes for one patient are not consistent with high quality of work. The occurrence of several diseases, cognitive disorders, depression (spread especially among patients of older age) need more time to contact with patient. The additional time is needed to implement the preventive activities and many other functional duties including making up of big amount of various documents. To implement this entire staff within the framework of present regulations is impossible. There is no way to finance the work of district therapeutist according the number of received patients. Under such an approach, it turned out that the more is the number of patients the better since more money the insurance companies will transfer to polyclinics and this will positively impact the physician financial situation. The opposite is positive - the lesser number of patients the better. This characterizes the effective work of physician and better implementation of preventive and curative measures. The present method of speed method of work of district therapeutist undoubtedly has a negative impact on population health and results in the emotional burning-out of physicians. The replacement of present system of health financing of primary health care activities is needed. The most part of physicians who participated in the survey propose the comeback to poll financing.
