THE RESULTS OF MONITORING OF NARCOTIZATION OF POPULATION IN CHUVASHIA

Abstract

In Chuvashia, the prevalence of drug addiction in population is 2.57 times lower as compared with Russia and Privolzhskiy Federal okrug. Nevertheless, the trend of annual 4-5% increase of number of patients with drug addiction and cases of intoxication by psychoactive substances as well is still present. The comprehensive assessment of narcologic care and medical social rehabilitation of persons with drug abuse is presented. The implementation of the Republic target antidrug programs and results of activities in the field of prevention of non-medical use of psychoactive substances are described.

THE REGIONAL STATISTIC MODELS OF TOBACCO SMOKING

Abstract

The representative sampling of 1027 smoking residents of Chuvashia (737 males and 290 females aged from 16 to 90 years, mean age — 35.8±13.4 years) was used to calculate the statistic models of tobacco smoking tobacco addiction, willingness for denial and break of tobacco smoking. The factorial model included nine indicators predicting tobacco smoking in the republic for 79.4% (motives and duration of tobacco smoking, age of respondent, number of smoked cigarettes per day, intensity of tobacco addiction, time of smoking first cigarette after wake up, desire to stop smoking tobacco). The valuable factors impacting the forecasting of tobacco smoking also included gender, university education, cigarettes brand, level of nicotine and tar in one cigarette. The established patterns can increase the effectiveness of anti-nicotine curative preventive activities in the region.

THE ROLE OF EXOGENOUS FACTORS IN THE DEVELOPMENT OF ACUTE TRANSIENT PSYCHOTIC DISORDERS

Abstract

The article considers the issue of the role of exogenous factors, alcoholization included in the development of acute transient psychotic disorders. The comparative study was carried out using two groups of patients with acute transient psychotic disorders without symptoms of schizophrenia (n=35, average age=30.5±10.3 years) and with symptoms of schizophrenia (n=35, average age=28.6±9.3 years). The clinical psycho-pathologic, clinical catamnesis and statistical methods were applied. The etiologic pathogenic aspects of acute transient psychotic disorders were discussed. The comparative analysis of premorbid characteristics of patients with acute transient psychotic disorders is presented. The unfavorable impact of alcoholization on course of disorders of schizophrenic spectrum is established.

ABOUT COMPARATIVE CHARACTERISTIC OF ALCOHOLIC AND NEUROLEPTIC CARDIOMYOPATHY

Abstract

The comparative analysis of clinical, electrocardiographic and morphologic manifestations of alcoholic (50) and neuroleptic (80) cardiomyopathies in comparison with control group (100 patients without cardiologic pathology). The statistic and morphometric research techniques were applied on macroscopic and microscopic levels. The results revealed no valuable differences between studied types of pathology. The conclusion is made that neuroleptic cardiomyopathy alongside with alcoholic cardiomyopathy is a single nosology unit (disease) with IDC (10th revision) code 142.7 ("Cardiomyopathy, conditioned by impact of medications and other external factors").

The program applies the cognitive behavioral approach and differentiated implementation of cognitive behavioral techniques and methods to patients with different types of character. In toto, at the sampling of 116 patients of main group one remission longer than one year was observed in 69.8% of patients and two year remission was observed in 30.2% 0f patients

THE SIGNIFICANCE OF MOTIVATION, STAGES OF READINESS TO BEHAVIOR CHANGES, FACTORS OF COMPLIANCE TO TREATMENT OF ALCOHOL ADDICTION

Abstract

The study was carried out to analyze motivation, stages of readiness to behavior changes, factors of compliance to treatment of alcohol addiction. The sampling included 50 male and female patients aged from 35 to 65 years with established diagnosis "alcohol addiction" (F10.2). The study revealed that first place among the motives which stimulated patient to visit physician for the purpose of formation of life-style of temperance was for "the issues at workplace - threat of discharge" with 55%. Among examined patients, 31.8% were at the first stage of readiness for behavior changes. The results approved that among patients with alcohol addiction taking into account the medical psychological aspects of compliance to treatment increases effectiveness of therapy and improve the quality of life.

Abstract

The article presents the model of studying the alcohol situation in Trans-Baikal Krai. The study established the characteristics of level and structure of prevalence of registered pathology and high percentage of alcohol disorders in youth people and patients of hospital somatic department. The main methodological approaches are presented to assess alcohol mortality. The social economic damage because of debauchery and alcoholism of population is calculated. On the basis of study results the system of monitoring was developed. This system can be implemented in other regions of the Russian Federation to collect the data following the unified technique.

THE APPROACHES TO TREATMENT OF WITHDRAWAL AND POST-WITHDRAWAL CONDITION IN NARCOLOGIC PATIENTS

Abstract

The article considers the issue of selection of effective method of treatment of alcoholism and drug addiction on the basis of analysis of actual situation in this field. The alternatives of pathogenic impact during therapy selection are proposed considering the known mechanisms of development of narcologic disease. The review of scientific data is presented concerning the directions of pharmaceutical effect on withdrawal and post-withdrawal condition of narcologic patients. The effectiveness of combined application of neuroleptics, benzodiazepines, antidepressants, and blockers of opium receptors (i.e. therapeutic model)is emphasized in case of treatment as an element of formation of lasting remission.

THE ANGIOSURGERY PATHOLOGY UNDER DRUG ADDICTION: THE ISSUE REVIEW

Abstract

The article presents the analysis of national and international scientific literature considering the issue of drug addiction and the complications related to intravascular introduction of various drug agents. The national tendency of increasing of percentage of persons with drug addiction is depicted. In Russia, according the official statistic data, 674 thousand citizens are taken various types of drug substances. In Moscow their number exceeds 35 thousand. Mainly, the drug addiction is a disease of young people. More and more adolescents and women are involved into process of narcotization. In Russia, about 60% of drug addicts are persons aged from 16 to 30 years. The ongoing increase of narcotization results in the increase of level of morbidity and mortality of young body-abled population. At that, among drug addicts 50.9% of terminal outcomes were caused by somatic disease. The data concerning different localization of vascular lesions is presented. In more than 50% the vessels of inguinal region are affected. Along with it, the lesions of shoulder and subclavicular arteries are presented too. The different considerations of researchers are proposed related to the mechanism of lesion of vascular wall impacting the different types of course of pathologic occurrences. It is noted that in drug addicts thrombosis of venous vessels has a non-embolic character. The causes of lower risk of pulmonary embolism in this category of patients are mentioned. The methods of radiologic diagnostic of vascular pathology in patients after intravascular introduction of drug substances are explained, including the comparative analysis of their reliability. Mostly, the factual cases of practice are described. Only several publications can be considered as a summary review. Hence, all negative factors occurring in case of lesion of blood vessels in drug addicts and their complications are rather actual issues of both angiosurgery and medicine in general. This condition testifies the need of comprehensive study of issues of diagnostic and treatment of this pathology. The benevolent outcome of pyo-septical complications in patients with drug addiction demands the development of rational approaches to diagnostic and treatment of this extremely severe category of patients.

Abstract

The choice of coping style at the first contact with oncological service determines the quality of life of oncological patients and impacts the effectiveness of anti-tumor therapy and ultimately the prognosis of disease. The alcoholization is the most frequent manifestation of non-adaptive coping strategies. The comprehensive study of coping strategies is needed to diagnose "the cancer" and further development of correction techniques targeting the decrease of the level of psychological deadaptation.

Abstract

The article considers the issue of hetero-aggressive behavior in women. The analysis of publication data is presented concerning epidemiology of psychic disorders diagnosed in women being in institutions of confinement. The relationship between aggressive behavior and presence of psychic pathology predisposing to aggression specifically addiction to psychoactive substances, is emphasized. The usage of psychoactive substances is an independent risk factor of aggression and results in adding more severety to present pathology hence supporting realization of aggressive behavior.

Abstract

The article deals with the analysis of rehabilitation activities in the system of care of individuals with psychoactive substances abuse as applied on the international level, in the Russian Federation and in countries of CIS. The principles and existing alternatives of rehabilitation care organization are described. The rehabilitation according the principles of therapeutic community applying multidisciplinary approach is considered as the most effective model of long-term rehabilitation. The deficiency of professional rehabilitation programs in the public sector of health care is noted as an issue of modern narcology. The ultimate actuality is given to the development of hospital substitute out-patient forms of rehabilitation as the most low-costs technologies.
